Serious Incident Reports
Human Rights First received a sample of 610 declassified incident reports, known as Serious Incident Reports (SIRs). These SIRs were released by the Department of the Army under a Freedom of Information Act request and cover the time period between June 1, 2004 and April 6, 2005. These reports provide an overall picture of the kinds of incidents private security contractors report on a day to day basis as well as how they report them – when they are actually reported.
Below are links to all 610 SIRs, arranged by month, as well as a list of acronyms and explanations for redacted material found within the reports. For more information on the SIRs or the incident reporting system, see Human Rights First’s report, Private Security Contractors at War: Ending the Culture of Impunity.
